Earmold Ordering Information
All earmolds and custom earpieces must be ordered through an audiologist, hearing aid dispenser, or licensed hearing care professional. Please call us at 800-842-8184 for a list of professionals located near you.
Hearing Care Professional Ordering Information
We will gladly provide you with catalogs, order forms, and mailing labels and/or boxes to start ordering with our company. We will accept any order for up to $50.00 on an open account. All we ask is that you send or fax us a completed credit application with your first order. All of our work begins immediately upon receipt of your impression(s), and most earmolds are fabricated and shipped the same day that they are received. For more information please refer to our Earmold catalog, Section: Laboratory Policies, pages 36 and 37.

We do not require minimum orders!
Ordering:
Orders may be placed by phone, fax, or e-mail. Be sure to include the catalog number, product name, quantity, and sizes and/or colors where applicable.

If there is a product not listed on our web site, please call us for availability.
Shipping and Handling:
Most orders placed by 3 PM will ship that same day. All orders over $10.00 will be shipped via UPS Ground. All orders under $10.00 will be shipped via first class mail. There will be a minimum-handling fee of $1.00 on all orders. Express shipping is available upon request. We cannot ship orders to a P.O. Box. You will receive e-mail verification on all orders placed via e-mail.
Invoicing Terms:
Our invoicing terms are net 30. An invoice will accompany all orders, unless we are instructed to send the invoice to another location. A statement will be mailed on the last weekday of the month. All new accounts must supply us with a credit application before an order will be processed on open credit. We accept VISA, MasterCard, and Discover for all accounts that do not wish to establish credit.
Returns:
We will gladly return any item if you are not fully satisfied with the merchandise that you receive. Items must be returned within 90 days in new condition. Shipping charges will not be credited.
