Earmold Ordering Information
All earmolds and custom earpieces must be ordered through an audiologist, hearing aid dispenser, or licensed hearing care professional. Please call us at 800-842-8184 for a list of professionals located near you. All of our work begins immediately upon receipt of your impression(s), and earmolds are fabricated and shipped within 1-3 business days.
Our invoicing terms are net 30. An invoice will accompany all orders, unless we are instructed to send the invoice to another location. A statement will be mailed on the last weekday of the month. All new accounts must supply us with a credit application before an order will be processed on open credit. Credit applications can be accessed under My Account > Forms and Agreements. We accept VISA, MasterCard, AMEX, and Discover for all accounts that do not wish to establish credit.
Mailed Impresseions
Our order forms can be printed on this website; click here for our printable form. Mailing labels and packaging can be requested via email or phone.
Scanned Impressions
If you have digital scanned impressions, please complete the fillable order form; click here for our fillable order form. Email your order form as an attachment along with your digital scanned impressions to info@greatlakesearmold.com. Please be sure to send your impression scans as an STL file.
Accessory Ordering Information
Ordering:
Orders may be placed by phone, fax, or e-mail. Be sure to include the catalog number, product name, quantity, and sizes and/or colors where applicable.
Shipping and Handling:
Most orders placed by 3 PM will ship that same day. All orders over $50.00 will be shipped via UPS Ground. All orders under $50.00 will be shipped via USPS. There will be a minimum-handling fee of $1.00 on all orders. Express shipping is available upon request. We cannot ship UPS orders to a P.O. Box. You will receive e-mail verification on all orders placed via e-mail.
Returns:
We will gladly return any accessory item if you are not fully satisfied with the merchandise that you receive. Items must be returned within 90 days in new condition. Shipping charges will not be credited.
